Individualized Care Plan (ICP)

Step 1: Click on ‘General Care/Report’ Orange Icon

Step 2: Click on ‘Individual Care Plan’, then proceed to click on ‘Add Individual Care Plan’

Important: Client must have an Active Admission ICP Assessment.

Step 3: Fill up ICP Form. Click “Save” or “Submit.

Table 23.2.3  tells you where the information from each section of the form is auto-populated from.

Step 4: Select the ‘magnifying glass button’ to view and the ‘pencil’ button to edit.
