Step 1: To add a Physiotherapy Assessment form, it is necessary to select the client first by searching for a client profile under Client > Client Listing on the left menu bar.
Step 2: On the client listing, scroll down to the client’s name or search for the client.
Step 3: Upon searching for the client’s name, click on the client’s name to view the client’s profile.
Step 4: Then select the 'General Care / Reports' icon.
Step 5: Select Physiotherapy Assessment and select Add Physiotherapy Assessment.
Step 6: Enter the relevant information and press Next or Save. (Save would mean that the form will not be submitted and the user will be able to continue to edit it at a later date/time)
Step 7: Once all the information has been entered, the user will be able to Submit the form.
Step 8: To view or edit the current assessment, select the ‘magnifying glass button’ to view or select the ‘pencil icon’ to edit the assessment.
