Add Medical Condition

Note: Patients can only be given chronic medication (13.1 Add Medication) if the patient has the relevant chronic condition. 

1. Go to the Patient Profile and click Medical History icon, indicated by the stethoscope icon.

2. To add medical conditions, select Add Patient Disease

To view each listing, select View indicated by the magnifying glass icon. To edit, select Edit indicated by the pencil icon.

3. Fill in all required fields and click Submit when done

4. To add family history medical conditions, select Add Family History Disease

Otherwise, to view each listing, select View indicated by the magnifying glass icon. To edit, select Edit indicated by the pencil icon.

5. Fill in all required fields and click Submit when done
