Adding Family History Diseases

In order to use this module, do make sure that the “Family History Diseases” function is enabled in the Settings (Refer to Section 2.1.1)

1. To add a medical condition, it is necessary to select the client first by searching for a client profile under Client > Client Listing on the left menu bar.

2. On the client listing, scroll down to the client’s name or search for the client.

3. Upon searching for the client’s name, click on the client’s name to view the client’s profile.

4. Then select the 'Stethoscope' icon.

5. The existing medical conditions can be seen under the header 'Family History Disease'. Click on the 'Add Family History Disease' button to add a new Family History Disease(s) not found in the existing list. 

6. Fill in all the required fields, then click the 'Submit' button.

   1.  Family Historical Disease

   2.  Year Diagnosed

7. The new Family History Disease will be added into the list of existing medical conditions, as shown below.
