Occupational Therapist Re-assessment

This form is for OT to reassess the client periodically to ensure that goals and interventions set in the ICP are effective. Re-assessment can be done half yearly or as and when required. Certain inputs from the latest ICP and / or re-assessment form will be auto populated in this form

 Step 1: Go to Client Profile. Click on the “General Care” Orange Icon.

Step 2: Click on “Occupational Therapist Reassessment Form” and Add an Occupational Therapist Reassessment Form.

Step 3: Fill up the form. 

Step 4: Click on “Save As Draft” or “Submit”.

Step 5: Click on the magnifying glass button to view the form. Click on the pencil icon to edit the form.
