Adding Indemnity Undertaking For Trial (Pre-Admission)

1. Users will be able to add Indemnity Undertaking for Trial.  They can do so by first selecting a client.  Click on Client > Client Listing

2. The user will be able to see the list of clients. Edit on a selected client by clicking on the client name. 
   
   

3. Click on the Nursing Care / Reports icon

4. Click on Pre-Admission > Indemnity Undertaking for Trial. 

5. Click on Add Indemnity Undertaking for Trial. 

1. The user can now click add a new Indemnity Undertaking for Trial form.

2. There are 4 sections of the form.  

   1. Type of Program to enrol, Client’s details, Emergency Contact

2. Care Person to Acknowledge

3. The Center to Acknowledge  

3. Once Submitted, the record will be stored in system.

The user is able to edit when required by clicking on the icon to re-edit the form.
